Parental Consent | Complete Player Lab

👨‍👩‍👧 Parent or legal guardian consent is required for athletes under 18 to participate in Complete Player Lab training.

I, the parent or legal guardian of the athlete named below, acknowledge that I have read and understand the Complete Player Lab training programs, policies, and terms. I provide informed consent for my child to participate in CPL basketball training.

Training Participation & Assumption of Risk

I acknowledge that basketball training and athletic development involve physical exertion and inherent risks, including muscle strain, injury, or other physical harm. I understand my child is participating in these activities voluntarily and at their own risk. I confirm my child is physically capable of participating in the training activities offered by Complete Player Lab.

Medical Responsibility

I confirm that my child is in good health and physically capable of participating in CPL training. I understand that I must inform Coach Q of any injuries, medical conditions, allergies, or physical limitations before my child participates in training sessions.

Release of Liability

By authorizing my child to participate in CPL training sessions, I release and hold harmless Complete Player Lab, Coach Q (Quinten Rock), CPL staff, and all affiliated individuals from liability or claims related to injuries sustained during training, except in cases of gross negligence.

Safety Compliance

I understand that my child is expected to follow all safety instructions and training guidance provided by the coach. I acknowledge that failure to follow safety instructions may result in removal from a training session.

Media & Photo Consent

I understand that CPL may capture photos or videos during training sessions for athlete development review, training demonstrations, coaching feedback, and marketing or social media purposes. I may request to opt out of media usage by notifying Coach Q in writing.

Financial & Cancellation Agreement

I agree to the Complete Player Lab Refund & Cancellation Policy, including the 24-hour cancellation requirement and session expiration policies. I understand that unused sessions do not roll over and will expire at the end of the billing period.

Terms of Service & Privacy

I have read and agree to the Complete Player Lab Terms of Service and Privacy Policy. I understand how my family's information will be collected, used, and protected.
